Kidneys not filtering efficiently. In diabetics, this rises later than Urine ACR — by the time creatinine is elevated, significant nephropathy is already present. This is why Gini tests Urine ACR first.
Low creatinine in older people or those with low muscle mass can be misleading — eGFR is more accurate.
