What Is COPD?

Chronic Obstructive Pulmonary Disease (COPD) is a long-term lung condition in which the airways become narrowed and the lung tissue is damaged, making it progressively harder to breathe out fully. It is caused mainly by long-term exposure to cigarette smoke, biomass and cooking-fuel smoke, occupational dusts and, in India, high levels of air pollution. COPD includes chronic bronchitis (persistent cough with sputum) and emphysema (destruction of the air sacs).

The airway obstruction in COPD is largely irreversible, which is why it cannot be cured β€” but it can be controlled very well. With the right diagnosis, inhaler therapy, pulmonary rehabilitation, vaccination and prompt treatment of flare-ups, most patients breathe more easily, have fewer hospital admissions and live active lives.

Common Symptoms

Diagnosis is confirmed with spirometry (a pulmonary function test). Dr. Katyal also uses chest X-ray or CT, oxygen saturation, and blood tests to assess severity and rule out overlap with asthma or heart disease β€” his dual training in pulmonary medicine and non-invasive cardiology is particularly valuable here.

COPD Staging (GOLD) β€” A Quick Guide

COPD severity is graded using the GOLD system, based on spirometry (FEV1) together with your symptoms and history of flare-ups. Staging guides which treatments you need.

GOLD Stage Airflow Limitation (FEV1) Typical Picture
GOLD 1 β€” MildFEV1 β‰₯ 80% predictedMild or no symptoms; often undiagnosed
GOLD 2 β€” ModerateFEV1 50–79% predictedBreathlessness on exertion; most patients seek help here
GOLD 3 β€” SevereFEV1 30–49% predictedMarked breathlessness, reduced capacity, frequent flare-ups
GOLD 4 β€” Very SevereFEV1 < 30% predictedSevere limitation; may need long-term oxygen; higher exacerbation risk

GOLD = Global Initiative for Chronic Obstructive Lung Disease. Staging is combined with symptom scores and exacerbation history to plan treatment.

How COPD Is Treated at Gini

A complete, stepwise plan tailored to your GOLD stage, symptoms and flare-up history β€” designed to keep you breathing well and out of hospital.

Inhaler Therapy

The foundation of COPD care β€” bronchodilators (LABA, LAMA) and, where indicated, inhaled corticosteroids. We spend time on inhaler technique and device choice, because poor technique is one of the most common reasons treatment "stops working."

Pulmonary Rehabilitation

A structured programme of supervised exercise, breathing techniques, education and nutrition guidance. Pulmonary rehab is one of the most effective interventions in COPD β€” it improves breathlessness, exercise capacity and confidence.

Oxygen Therapy

For patients with persistently low oxygen levels, long-term oxygen therapy can prolong life and reduce strain on the heart. Dr. Katyal assesses your oxygen needs carefully β€” the right flow at the right time matters, and too much oxygen can be harmful in COPD.

Vaccination & Smoking Cessation

Stopping smoking is the single most important step to slow COPD. We support you with a cessation plan, and recommend influenza and pneumococcal vaccination to reduce the infections that trigger flare-ups.

Acute Exacerbation Management

A flare-up needs prompt action β€” increased bronchodilators and nebulisation, a short course of steroids, antibiotics when infection is suspected, and controlled oxygen. Severe exacerbations are managed with non-invasive ventilation (BiPAP) and, when needed, intensive care.

Heart–Lung Overlap Review

Breathlessness in COPD is sometimes worsened by co-existing heart disease. Dr. Katyal's training in non-invasive cardiology lets him assess both together, so the true cause of your symptoms is addressed.

When COPD Needs Hospital & ICU Care

Most COPD is managed in the outpatient clinic. But a severe exacerbation or respiratory failure can be life-threatening and needs admission. Seek urgent hospital care if you have any of the following:

πŸ”΄ Severe breathlessness at rest, unable to complete a sentence
πŸ”΄ Drowsiness, confusion or unusual sleepiness
πŸ”΄ Bluish lips or fingertips, or SpO2 below 90%
πŸ”΄ No improvement despite home inhalers and nebulisation

Gini Advanced Care Hospital provides intensive-care support for severe COPD, including controlled oxygen, non-invasive ventilation (BiPAP) and, when required, invasive mechanical ventilation. Our ICU is led by Dr. Rahul Katyal, Director of Critical Care, with round-the-clock intensivist cover, an in-house lab for rapid arterial blood gas and electrolyte results, and direct connection to the emergency department for fast admission. Dr. Katyal manages the full spectrum of COPD β€” from first diagnosis and inhaler optimisation in the clinic to non-invasive ventilation and intensive care for severe exacerbations. His dual training in pulmonary medicine and non-invasive cardiology makes him especially well suited to patients whose breathlessness involves both the lungs and the heart, a common overlap in older COPD patients.

COPD β€” Frequently Asked Questions

Can COPD be cured?+
COPD cannot be cured because the airway damage is largely irreversible, but it can be controlled very well. Stopping smoking, correct inhaler technique, pulmonary rehabilitation, vaccination and prompt treatment of exacerbations slow the decline, reduce hospital admissions and greatly improve breathlessness and quality of life.
What is the difference between COPD and asthma?+
Asthma usually begins in childhood, is driven by allergy or inflammation and causes reversible airway narrowing. COPD usually develops after age 40, is caused mainly by smoking or biomass and occupational smoke exposure, and causes largely irreversible obstruction. A spirometry (PFT) with a detailed history distinguishes the two, and some patients have overlap of both.
When does a COPD patient need hospital admission?+
Admission is needed for a severe exacerbation with marked breathlessness at rest, confusion or drowsiness, bluish lips or fingertips, an SpO2 below 90%, a very high breathing rate, or symptoms not improving despite home inhalers and nebulisation. These patients may need controlled oxygen, non-invasive ventilation (BiPAP) or ICU care. Call +91 82888 43800.
What is a COPD exacerbation?+
An exacerbation (flare-up) is a sustained worsening of cough, sputum and breathlessness beyond the usual day-to-day variation, often triggered by chest infection or air pollution. Treatment includes increased bronchodilators, a short course of steroids, antibiotics when infection is suspected, and controlled oxygen or ventilatory support in severe cases.
Does Gini Hospital have ICU support for severe COPD?+
Yes. Gini Advanced Care Hospital in Mohali provides intensive-care support with invasive and non-invasive ventilation for severe COPD exacerbations and respiratory failure, led by Dr. Rahul Katyal, Director of Critical Care. Call the 24/7 emergency line on +91 82888 43800 for urgent breathlessness.
