Who is Eligible for a CGHS Card?

CGHS covers central government employees and pensioners only — not state government.

Step-by-Step: Apply Online at cghs.nic.in

Visit the CGHS Portal
Go to cghs.nic.in → Click "Apply for New CGHS Card". Create a beneficiary account with your government email ID or Aadhaar-linked mobile number.

Fill the Application Form
Enter your personal details, service details (department, designation, pay level), and residential address in Chandigarh zone. Add all dependants you want covered.

Upload Required Documents
Upload scanned copies of: service/pension ID, Aadhaar, pay slip or PPO, passport photos, and dependant proof documents (see document list below).

Pay Subscription Online

Monthly subscription is deducted from salary (employees) or pension (pensioners).

Adding a Dependant After Card Issue

You can add a new dependant (e.g., newborn baby, newly wed spouse, newly eligible parent) at any time via the CGHS portal or at your Wellness Centre.

Required: updated CGHS card application, dependant proof documents (birth certificate, marriage certificate, Aadhaar of dependant), and your original CGHS card for endorsement.

Processing time: 2–3 weeks. In urgent cases (newborn requiring treatment), the Wellness Centre can issue an OPD slip pending card update.

Address Change / Transfer

If you have transferred to a new CGHS city (e.g., from Delhi to Chandigarh), apply for an address change and Wellness Centre reallocation at cghs.nic.in. Your new Wellness Centre in the Chandigarh zone will be assigned.

Documents: transfer order, new address proof, original CGHS card. You can also walk into the AD CGHS Chandigarh office (Kendriya Sadan, Sector 9-A, 0172-2740716).

Lifetime CGHS Card for Pensioners

Central government pensioners are issued a lifetime CGHS card — it does not expire and does not need renewal. It remains valid until the pensioner passes away, after which it can be converted to a surviving spouse card (also lifetime).

Pension is deducted at a fixed monthly rate — even if you are admitted to hospital 3 times a year, you continue paying the same subscription. There is no "usage limit" or claim ceiling for IPD cashless treatment at empanelled hospitals under CGHS.

Can I apply for a CGHS card online?+
Yes. The application process is online at cghs.nic.in. You fill and submit the form online, pay subscription online, then visit your Wellness Centre once for document verification. The card is issued within 2–4 weeks of verification.

No. Pensioners must choose either FMA or CGHS — not both. If you are currently receiving FMA, you must surrender it before enrolling in CGHS. The choice is usually worth making if you anticipate a major procedure like knee or hip replacement — CGHS covers it cashless, while FMA typically amounts to ₹500–1,000/month and provides no cashless cover.
How long does it take to get a CGHS card?+

I am a state government pensioner — can I apply for CGHS?+
